VERIFICATION OF BOARD CERTIFICATION

Managed care organizations, hospitals and health care agencies: To verify a physician's Board Certification, please submit a request in writing along with a signed release form from the physician (form must be dated no more than one year prior) to the ABPS Executive Offices AND the required fee via fax or mail to the contact below. ABPS responds to verification requests within 14 days from the receipt of the request. The ABPS attestation fee for all written verification requests received after January 15, 2010 is $50 per individual verification. The fee may be paid by check or credit card (Visa, MasterCard or American Express in US funds). Credit card data must include the name as it appear on Card, the Amount, the credit card number and the expiration date.
