Board Certification Verification

About Verification

Board Certification Verification provides an official written attestation of a physician’s board certification(s) including the specialty, certification expiration date and certificate number.  Official attestation letters, printed on ABPS letterhead, are sent via U.S .mail.  Upon request, a faxed copy of the attestation letter is sent when the official letter is mailed.

Verification Requests

Verification requests can be submitted via fax or mail.  All requests must include:

  • an ABPS Verification Request Form
  • a written letter of request (see requirements below), and
  • a release form from the physician dated within the last year

Letters of Request:  All letters of request must be printed on official letterhead of the requesting organization, or agency to include the name and date of birth of the physician.  Letters of request must also include the name and signature of individual making the request.

Attestation Fee:  An attestation fee of $50 per verification is due at the time of the request.  The attestation fee is waived for State Medical Boards.  Organizations needing a Federal Tax ID number and/or Dunn & Bradstreet number can look up ABPS on the Central Contractor Registry (CCR).  We are listed as “American Association of Physician Specialists, Inc.”

Payment Methods: The attestation fee can be paid by check or credit card (VISA, Master Card or American Express).  Payments are accepted in U.S. funds only.  Payment may be submitted via the phone, mail or fax.

Complete verification requests are mailed within 14 days from the date of receipt.
